Q: Dr. Epsy, I have questions about cheekbone reduction. What sides of the cheekbone can shave and minimized? I want you not to stand out from my face.

A: Pomulus reduction surgery is performed by osteotomies, usually of the chief prior body of the cheekbone, as well as the thin posterior zygomatic arc, to allow movement in the entire cheekbone complex. The reduction of the cheek bone is not performed when shaving due to limited access, the thinness of the bone and the appearance of the soft tissue cheek sunk by the elimination of all the unions of the soft tissues.

High cheekbones are often considered a desirable facial feature, but for some individuals, overly prominent or wide cheekbones (particularly in the zygomatic arch area) may cause facial imbalance or a broader facial appearance. One common question patients ask is: “Can cheekbone reduction be done by shaving the bone?” The answer is nuanced and depends on your anatomy and goals.

Understanding Cheekbone Reduction

Cheekbone reduction, also known as zygomatic reduction surgery, is a procedure designed to decrease the width and prominence of the cheekbones. This can improve facial balance and provide a softer, more contoured appearance. There are two main techniques used:

  1. Zygomatic Arch Osteotomy (Bone Cutting)

  2. Zygoma Shaving (Bone Contouring)

Cheekbone Shaving: Is It an Option?

Yes, cheekbone reduction can sometimes be performed through shaving or contouring the outer surface of the zygomatic bone. This method involves smoothing down the bone using surgical burrs or rasps to subtly reduce its projection. However, there are limitations:

When Is Bone Cutting Necessary?

For more pronounced cheekbones or a wider facial structure, zygomatic osteotomy is usually required. This involves surgically cutting the zygoma and repositioning it inward to reduce the facial width and projection. This technique allows for more significant reshaping and narrowing of the midface.

Which Option Is Right for You?

Whether shaving alone is sufficient depends on:

  • The degree of cheekbone prominence.

  • Your facial proportions and symmetry.

  • Your aesthetic goals and tolerance for downtime.

A thorough evaluation by a board-certified facial plastic or craniofacial surgeon is essential. Advanced 3D imaging and facial analysis can help determine the most effective and safest approach.
